Our Care Manager works with the health care providers to educate and empower patients to reach their goals and help patients and their families cope with complicated health issues. The Care Manager provides a range of services to address barriers to care, including those related to language and culture, communication and transportation.  Our Care Manager works with patients through telephone calls and in-person visits to create a care plan that will best address individual health needs.

Care Management services include the following:

  • Appointment scheduling and follow-up
  • Health education
  • Transitions support
  • Referrals
  • Self-management support
  • Culturally competent and linguistically appropriate care
  • Transportation assistance
  • Program eligibility and enrollment assistance
  • Connection to other community-based or social services

About Our Care Manager
Kate Drackett grew up in a small town in central Wisconsin, and attended Evangel University in Missouri for her undergraduate degree. After working for several years at an HIV/AIDS clinic, Kate decided to pursue higher education and attended Missouri State receiving a Master’s degree in Social Work. Kate relocated to Colorado and is a Licensed Clinical Social Worker in the state of Colorado. Kate has worked in multiple medical practices as an integrated behavioral health clinician and is dedicated to providing quality mental health care. Kate enjoys spending time with her family: building Legos with her son, trying to keep up with her daughter, and hiking with her husband.
